NY, 6Division of Gastroenterology and Hepatology, Department of Medicine, New York University, New York, NY, USA: The use of sirolimus as an alternative to calcineurin inhibitors for posttransplant immunosuppression is associated with a variety of inflammatory conditions. In this report, we describe the case of a 34-year-old man who developed abnormal liver tests 6 years after live-donor kidney transplantation and 5 years after initiation of sirolimus-based immunosuppression. Elevated aminotransferase levels persisted after withdrawal of potentially hepatotoxic medications, and serologic evaluation for viral hepatitis, autoimmune disease, and genetic disorders was unrevealing. Liver biopsy revealed prominent hepatocellular injury associated with a mixed inflammatory infiltrate and liver tests normalized within 2 weeks of discontinuation of sirolimus. In this report, we review previous reports of sirolimus hepatotoxicity and propose a unifying hypothesis for the various inflammatory conditions that have been attributed to sirolimus.Keywords: sirolimus, immunosuppression, transplant, inflammatory conditions, hepatotoxicity. The relationship between adults and children in liberal democracies is based on two flawed assumptions that are widespread: first, that childhood is an impediment, a passing phase of impaired maturity; and second, that children benefit from the proliferation of rights ascribed to them. Social institutions, and particularly the education system, are correspondingly misconstrued. This article focuses on the combined effect of vulnerability and autonomy as they construct contemporary childhood. I conclude that adults' obligations rather than children's rights are the appropriate (...) social, political and educational basis for adult society's relations with children. (shrink)

A distinction is commonly drawn between continuous sedation until death and physician-assisted suicide/euthanasia. Only the latter is found to involve killing, whereas the former eludes such characterization. I argue that continuous sedation until death is equivalent to physician-assisted suicide/euthanasia in that both involve killing. This is established by first defining and clarifying palliative sedation therapies in general and continuous sedation until death in particular. A case study analysis and a look at current practices are provided. This is followed by a (...) defense of arguments in favor of definitions of death centering on higher brain (neocortical) functioning rather than on whole brain or cardiopulmonary functioning. It is then shown that continuous sedation until death simulates higher brain definitions of death by eliminating consciousness. Appeals to reversibility and double effect fail to establish any distinguishing characteristics between the simulation of death that occurs in continuous sedation until death and the death that occurs as a result of physician-assisted suicide/euthanasia. Concluding remarks clarify the moral ramifications of these findings. (shrink)

It is often alleged that liberalism and nationalism are mutually antagonistic in theory and practice. Max Weber is a good example, the dominant interpretation maintains, as his political thought betrays its liberal foundation by embracing an ardent nationalism that was popular in Wilhelmine Germany. Weber was, in short, a nationalist, and thus illiberal, political thinker. Against this conventional wisdom I argue that Weber's liberal nationalism cannot be placed squarely in the authoritarian, ethnic tradition of German nationalism, and its idiosyncrasy becomes (...) evident once Weber's twofold political project of revivifying a robust civil society while imbuing it with the spirit of public citizenship is foregrounded more clearly. Thus recast, Weber's political thought reveals a strong affinity with that of Tocqueville and Mill, especially in their similar concern with moral personality and political maturity in a mass democracy. This volume contains a critical edition and annotated translation of three previously unpublished and virtually neglected commentaries of Averroes on Aristotle. The edition is based on the two extant Judaeo-Arabic MSS which were collated with the thirteenth-century Hebrew translation of Jacobben Makhir and the sixteenth-century Latin translation. In addition, there is an introduction that includes a discussion of the teaching of the text and indices of names and titles and technical terms. It is shown that the jet mechanism derivable from the Lorentz deformation picture leads to a nearly constant average jet transverse momentum. It is pointed out that this is consistent with the high-energy experimental data. It is pointed out further that this result strengthens the physical basis for the minimal time-energy uncertainty combined covariantly with Heisenberg's space-momentum uncertainty relation.

On the surface, this book is an interesting and well-written commentary on Marx’s thought, with the emphasis on the writings of the young Marx. It is much more than that, however, as it is clear that Axelos sees this study as a prologue for an attempt to transcend Marx by means of a correction of certain aspects of Marx’s thought.

Pragmatics, Truth and Language is a collection of 21 papers, 8 previously published elsewhere, a companion volume to Martin's Events, Reference and Logical Form and Semiotics and Linguistic Structure ; it will be of interest to linguists as well as philosophers of language. Many of the papers are quite technical, but Martin writes very clearly, and the book should be accessible to advanced graduate students.

Juvenal opens his eighth Satire with the question stemmata quid faciunt?, supplies an answer in line 20, nobilitas sola est atque unica virtus, and devotes the rest of the poem to exhorting his addressee to virtuous activity, both by negative exempla drawn from the degenerate nobility and by positive exempla drawn from the plebs, novi homines and the like. In lines 39–70 he addresses one particularly self-important noble and attempts to deflate his bombastic pride: in 56–67 he adduces an extended (...) illustration from the animal world, apparently such as was common in the schools of rhetoric. In Satire 3, Umbricius states his intention to leave Rome and delivers a long explanation of his decision, an explanation which develops into an invective against life in Rome. In the lines quoted above, Umbricius lists the ‘skills’ which are essential for success at Rome, ‘skills’ which he does not possess. The list comprises various mendacious, nefarious and criminal activities; Umbricius' stated inability to undertake such activities reinforces his claim to be a simple, honourable man. In this list is his (...) claim ‘I have never examined frogs’ entrails'. TLL glosses this passage ‘’, the view taken by Friedländer too. Courtney takes a different view: ‘Presumably not for poisoning, which inspicere would hardly suit, but of divination … of an oriental type.’. (shrink)
